Respirator Medical Clearance for Self-Employed Contractors

No employer program? Get cleared on your own.

A general contractor or job site wants proof of respirator medical clearance, but there is no company program to send you through. Complete the OSHA Appendix C questionnaire online, get reviewed by a licensed healthcare professional (PLHCP), and send your certificate the same day.

  • Good for 12 months
  • $22 - no hidden fees
  • No clinic visit
Get Cleared - $22

Renewal only $19

Why self-employed workers get asked for clearance

OSHA's rules are written for employers. According to OSHA, self-employed workers with no employees are not covered by the OSH Act — so OSHA itself does not require you to have medical clearance.

The people you work for often do. General contractors and site owners commonly require proof of respirator medical clearance from everyone who wears a respirator on their site, including subcontractors and independent contractors. Without it, you can be kept off the job.

If you have employees of your own, the OSHA respiratory protection standard (1910.134, and 1926.103 in construction) applies to you as an employer, including providing medical evaluations to the employees you require to wear respirators.

When contractors usually get asked

Most requests come at the same moments:

Onboarding with a new general contractor

Site safety paperwork often lists respirator medical clearance alongside fit test and training records.

Dusty or coatings work

Concrete, drywall, demolition, painting, and solvent work where the site requires respirators.

Annual renewal

Clearance is good for 12 months, and many sites ask for a current certificate each year.

What you send the GC

Approval gives you everything a site safety manager usually asks for:

  • A PDF certificate of OSHA 1910.134 medical clearance in your name
  • A wallet card
  • A QR code anyone on site can scan to verify your clearance
  • A receipt for your business records

Self-employed respirator clearance FAQ

I'm a self-employed contractor. How do I get OSHA respirator medical clearance for myself?

Register online, pay $22, and answer the OSHA Appendix C medical questionnaire. A licensed healthcare professional (PLHCP) reviews your answers; most people are approved instantly and download a certificate and wallet card right away.

Does OSHA require clearance if I work for myself?

According to OSHA, self-employed workers with no employees are not covered by the OSH Act. In practice, general contractors and site owners often require proof of clearance before you work on their site.

Does the certificate travel with me between job sites?

Yes. It is issued to you, not to an employer, and is valid for 12 months. Each GC decides what it accepts; the QR code lets them verify it instantly.

Do I still need a fit test?

If the site requires a tight-fitting respirator, usually yes. Medical clearance comes first; a fit test checks that your specific respirator seals to your face.
